Girls physicals will be on Tuesday, May 22, starting at 4:00 and boys physicals will be on Wednesday, May 23, also starting at 4:00, both in the gym.

The Adventist Health form is available on the Tokay High School website under Athletics/Sports Clearance.  Please fill the parent/student portion and take it with you.  Wear loose fitting and comfortable clothing.  Depending on participation, there will be some wait time, so parents can drop off students to be picked up later, if they wish.

Along with the online registration, the completed physical will clear you for opportunity to play any sport throughout the year. DON’T MISS OUT ON THIS OPPORTUNITY!

THIS IS A MUST IF YOU ARE PARTICIPATING IN SUMMER CONDITIONING.
